ANALISIS PENGARUH PERSEPSI DOKTER TENTANG FORMULARIUM TERHADAP KETAATAN PENULISAN RESEP SESUAI DENGAN OBAT DALAM FORMULARIUM DI RUMAH SAKIT DAERAH KABUPATEN KUDUS TAHUN 2004

Abstract

Although annual drug budget at Kudus district Hospital was increase, it is not filled up the drug requirements in Kudus district Hospital. It's cause not only the increase of drug price but also many prescribing which does not get along with the formulary manual. The doctor's negative perception in the Kudus district hospital may be caused and encouraged by the prescription whict does not get along with the formulary manual. The goal of this study was to find out the the effect of doctors perception about the formulary manual toward the Obedience of Prescribing According to The formulary manual. The Obedience of Prescribing is a behaviour. Perception and obedience of prescribing is influenced by the internal and external factors. Internal factors consist of working term and the school medicine term while the external factors consist of the quantity of formulary system information. Design of the study was observational cross sectional with the total sample of 30 doctors at the Kudus district Hospital, the study uses the questioner. The study found : 1. There is no correlation between external and internal factors with the doctor' perception about the formulary manual(p>0.05), except the quantity of information of formulary system has significant correlation with the perception of the usedfull formulary manual. 2. There is no significant correlation between internal and external factor wit the obedience of prescribing 3. There is a significant correlation between the perception about the formulary manual with the obedience of prescribing(p<0.05), except the perception about the content of formulary manual(P>0.05) 4. Logistic Regression analysis suggest that the main variable which influences the perception about formulary manual is the quantity of formulary system information(p 0.049 r 0.873), while the main variable which influences the obedience of prescribing is the doctor's perception variable about the usedfull of the formulary manual(p 0.029, r 0.338) The suggest, it is necessary that the effort to increase the doctors obedience of prescribing according to the formulary manual, the effort can be done to increase the obedience of prescribing are to increase the frequently of giving the information obout the usefull of formulary system and giving reward and punishment Obat merupakan salah satu komoditas yang amat penting bagi rumah sakit., karena komponen terbesar biaya di RS adalah obat. Meskipun tiap tahun anggaran obat bertambah, namun anggaran tersebut belum dapat memenuhi kebutuhan obat RSD Kudus. Hal ini disebabkan selain naiknya harga obat juga oleh karena banyaknya penulisan resep obat di luar formularium. Kemungkinan penyebab dan pendorong peresepan di luar formularium adalah adanya persepsi negatif dokter RSD Kudus tentang formularium. Penelitian ini bertujuan untuk mengetahui pengaruh persepsi dokter di RSD Kudus tentang formularium RSD Kudus terhadap ketaatan penulisan resep sesuai obat yang tercantum dalam formularium. Ketaatan Penulisan resep adalah perilaku. Persepsi dan Ketaatan Penulisan resep dipengaruhi oleh faktor internal dan eksternal. Faktor internal terdiri dari Lama Bekerja di RSD Kudus Lama Pendidikan dokter,faktor eksternal terdiri dari Kuantitas informasi formularium. Desain penelitian adalah obseivasional crossectional dengan jumlah sampel 30 orang dokter di RSD Kudus, penelitan menggunakan kuesioner dan wawancara mendalam. Data dianalisis dengan univariat,bivariat dan multivariate Hasil analisis menunjukkan 1. Tidak ada hubungan antara faktor eksternal dan internal dengan persepsi dokter tentang formularium RSD Kudus(p>0.05), kecuali kuantitas informasi ada hubungan yang bermakna dengan persepsi manfaat formularium(p<0.05) 2. Tidak ada hubungan yang bermakna antara faktor internal dan eksternal dengan Ketaatan Penulisan resep 3. Ada hubungan yang kuat dan bermakna antara persepsi tentang formularium dengan Ketaatan Penulisan resep(p<0.05) kecuali persepsi tentang isi formularium(p>0.05) 4. Analisis Multivariat dengan regresi logistik menunjukkan bahwa variabel utama yang mempengaruhi persepsi tentang formularium yaitu kuantitas informasi formularium(p 0.049 r 0.837), sedangkan variabel utama yang mempengaruhi Ketaatan Penulisan resep adalah variabel persepsi responden tentang manfaat formularium RSD Kudus(p 0.29 r 0.338) Hashl penelitian ini menunjukkan adanya pengaruh persepsi dokter tentang proses penyusunan dan manfaat formularium RSD Kudus terhadap ketaatan penulisan resep. Saran, perlu upaya meningkatkan ketaatan dokter dalam penulisan resep obat yang sesuai dengan formularium RSD Kudus, upaya yang bisa dilakukan untuk meningkatkan ketaatan penulisan resep antara lain meningkatkan kekerapan pemberian informasi tentang manfaat formularium serta pemberian reward and punishment
