Abstract

Background : Perennial allergic rhinitis (PAR) as a chronic disease the symptoms may impair patients quality of life (QOL), therefore clinical assessment of treatment result based on both symptoms and patient's QOL improvement. Ob ective : To compare the effectiveness of Chlorpheniramine maleat (CTM) and Loratadine (Alloris) for treating PAR based on clinical symptoms and patient's point-of-view assessment using the quality of life questionnaire (QOLQ). Study design : Randomized double blind clinical trial Methods : PAR patients, 15 - 55 years old, who fulfilled the inclusion and exclusion criteria, assesed their QOL before and after taking the pills for 7 days using the QOLQ that contains 6 domains. They were randomly allocated into two groups of treatment CTM (2 x 4 mg) and Loratadine (1 x 10 mg). Clinical symptoms was assessed by 4L point scale. T - test was used for comparing mean of QOL score ( total and each domain) and total clinical symptoms score. Results : 63 PAR cases, 32 (CTM group) and 31 (Loratadine group), with 7 cases drop-out (11,11%). The total-QOL mean score of the Loratadine group was 18,33 + 17,22 compared with CTM group 16,50 + 11,32. When using t-test the difference was not significant (p> 0,05, p= 0,654). There were no difference for each domains sleep (p=0,814), nose symptoms (p=0,624), non- nose/eye symptoms (0,845), practical problems (p=0,847), activities (p=0,287), and emotion (p=0,970). The difference of Total clinical symptom score for both groups was not stastistically significant (p> 0,05, p = 0,192) Conclusion : There was no significantly difference between Loratadine and CTM in improving QOL of PAR (p = 0.654), and in total symptom score (p = 0,192) Latar belakang : Rinitis alergi perenial (RAP) adalah penyakit kronis dimana gejalanya dapat mempengaruhi kualitas hidup (KED penderitanya, sehingga penilaian hasil terapi didasarkan pada perbaikan gejala klinis dan KII penderita. Tujuan : Membandingkan efektifitas Chlorpheniramine maleat (C11,4) dengan Loratadine (Alloris) untuk terapi RAP berdasarkan gejala Minis dan penilaian penderita memakai kuesioner KH (KKH) Rancanaan penelitian : Uji Minis acak tersamar ganda Metodologi penelitian : Penderita RAP, usia 15 - 55 tahun , yang memenuhi kriteria inklusi dan eksklusi, menilai KH-nya sebelum dan setelah pemberian obat (selang 7 hari) dengan menggunakan KKH berisi 6 domain. Kasus dialokasikan secara acak dalam kelompok terapi CTM (2 x 4 mg) dan Loratadine (1 x 10 mg). Gejala klinis dinilai dengan menggunalour sekor 4 angka. Uji-t digunakan untuk membandingkan rerata sekor KI1 (total dan masing-masing domain) serta sekor gejala total . Hasil uenelitian : Didapat 63 kasus, 32 dalam kelompok CTM dan 32 kelompok Loratadine, dengan 7 kasus (11,11%) yang "drop out". Rerata sekor KII- total kelompok Loratadine adalah 18,33 + 17,22 dibanding kelompok CTM 16,50 + 11,32, yang secara statistik memakai uji-t perbedaannya tidak bennalum (p> 0,05; p = 0,654). Demikian juga halnya untuk masing-masing domain : Tidur (1)=0,814), Hidung (p=0,624), Alergi lain (p=0,845), Masalah praktis (p=0,847), Aktifitas (p,287), dan Emosi (p=0,970). Sekor gejala total antara kedua kelompok juga tidak berbeda bermakna secara statistik (p > 0,05, p = 0,192). Kesimpulan : Loratadine dan CTM memiliki efek yang tidak berbeda bermakna secara statistik dalam memperbaiki KH penderita RAP (p=0,654), dan juga sekor gejala total (p=0,192).
