Abstract

Background In performance of hernodialysis ( HD ), it necessary to have cufficient great fund, moreever in HD tool. The repeatedly use of artificial kidney gives advantages cost efficiency and increased bio-compatipibility and it also give disadvantages, decreased efficiency, disinfectant contamination / bacteria, infectiont transmission. Objective The goal of this study is to understand the repeatedly used of dializer againnst urea-n and Urea-n reductiont ratio and understand how many times dializer was used repeatedly and properly. Metodology This research was conducted by comparative research design , statistical test using multiple correlative regression test. Location research was in Dr Kariadi hospital hemodialysis unit from September 1999 to January 2000. Samples is chronic renal failure patient was HD in hemodialysis unit Dr Kariadi hospital including the inclusive criteria. Ureum was measured before and after HD. Result 21 CRF paient with HD treatment were examined, 16 ( 76,19%) males and 5 ( 23,81 % ) females. The repeatedly use was maximum 7 x and mean 5,1 x because the tool was no longer proper to use. New clirens urea-n dialyzer in vivo average was 126,15 ml/ sec. Average new dialyzer compartement content was 70 ml, third use was 67,72 ml, decrease was 3,2 %. Estimation of relationship between repeatedly use of dialyzer was just obtained. The repeatedly use did not decrease clirens urea-n dialyzer multiple R =-0,16108 p= 0,485 was seen from the final use and multiple R= -0,0242 p= 0,9170 was seen from average / tool. And repeatedly use did not decrease URR multiple R= - 0,2475 p=0,279 was seen from final use and multiple R= -0,0896 p=0,699 was seen from average / tool. The decrease in clirens urea-n started to take palce at fourth use ( R3 ) with B unstandardized coefficient as much as — 0,184 Conclusion The repeated use of dialyser did not decrease clirens urea-n and dialyzer URR Rate, the should be stopped after fourth use ( R3) because decrease in clirens urea-n and URR occured Lata r belakang Dalarn pelaksanaan hemodialisis ( HD ) diperlukan biaya yang cukup besar. Yang paling mahal adalah ginjal buatan dialiser. Penggunaan ginjal buatan pakai berulang rnernberikan keuntungan penghematan biaya dan peningkatan biokompatibilitas, serta memberikan kerugian kemungkinan penurunan efisiensi, kontaminasi desinfektan dan transmisi infeksi. Tujuan Penelitian ini bertujuan untuk mengetahui pengaruh penggunaan dialiser berulang terhadap kliren urea-n dan URR, serta mengetahui sampai berapa kali dialiser masih layak digunakan berulang. Metodologi Disain penelitian komparatif, uji statistik menggunakan uji regresi korelasi berganda. Waktu dan tempat : Selama bulan September 1999 sampai dengan Januari 2000, di Unit hemodialisis RS Dr Kariadi Semarang. Sampel : penderita gagal ginjal kronik yang menjalani HD di RS dr Kariadi yang memenuhi kriteria inklusi. Diperiksa ureum pre dan post HD, dihitung dengan rumus. Hasil Telah diteliti 21 penderita, 16 ( 76,19 %) laki-laki dan 5 ( 23,81 % ) perempuan. Pemakaian berulang maksimal 7 kali, mean 5,1 karena alat sudah tidak layak lagi dipakai. Kliren urea-n dialiser baru in vivo rata-rata 126, 15 ml/men dan in vitro 171 ml/men. Rata-rata isi kompartemen darah dialiser baru 70 ml, pemakaian ketiga 67,72 ml, terjadi penurunan 3,2 %. Penilaian hubungan antara pernakaian dialiser berulana dengan baru didapatkan : pemakaian berulang tidak menurunkan kliren urea-n multipel R = - 0,16108 p = 0,485 dilihat pemakaian terakhir dan multipel R= - 0,0242 p=0,917 dilihat rata-rata per alat. Pemakaian berulang tidak menurunkan URR multipel R---0,2475 p= 0,279 dilihat pemakaian terakhir dan multipel 12--- -0,0868 p= 0,699 dilihat rata-rata per alat. Penurunan kliren urea-n mulai terjadi pada pemakaian ke 4 ( R3) dengan B unstanclarized coefficient —0,0 868 dan penurunan URR mulai terjadi pada pemakaian ke 4 (R3) dengan B unstandarized coefficient sebesar — 0,184 Kesimpulan Pemakaian dialiser secara berulang tidak menurunkan kliren urea-n dan angka URR dialiser, pemakaian sebaiknya dihentikan setelah pemakaian ke 4 (R3) karena mulai terjadi penurunan kliren urea-n dan URR.
