Abstract

Urinary Tract Infection ( U T I ) is a common type of infections disease that still affects about 5 - 6 % of the whole general population. It is found in private practice and hospitals and it may attack every body, regardless of ethnicity and residence, but it depends more on the victim's age and gender. The U T I diagnosis is made by referring to Kass criteria, i.e, significant bacteriuria of > 10 5 CFU / mL and Leukosituria of > 5 / HPF for men and of >15 / HPF for women, and asymptomatic patients. It is believed that the U T I - causing bacteria have the antigent that can produce cross - reaction with the blood groups containing Anti - B isoaglutinin. The absence of Anti - B isoaglutinin and the structural resemblance of E. Coli Bacteria's antigent, the most common cause of U T I, with that of the antigent of both B and AB blood groups give a higher U T I risk to the B and AB blood groups, as opposed to the A and 0 groups. A study on the U T I risk among the B and AB blood groups was conducted in the Pathology Clinic of Dr Kariadi General Hospital in Semarang, using the analytical and observational methods with the cross - sectional approach within a period of 5 months, from July 1997 to November 1997. Taking 336 leucosituria urine samples from male and female , patients without any clinical complaints, all coming from the ABO blood groups, aged 15 - 50 years, the researcher conducted the bacterial culture examination. The examination revealed that 214 ( 63,7 % ) were UTI negative and 122 ( 36,3 % ) were UTI positive, and the average age of the UTI patients was more commonly found among-the productive ages ( 40 years ). Of the UTI group, most cases were found among the B blood group ( 48,4 % ) and the AB blood group ( 48,5 ) with the negative - gram intestinal bacteria, especially E. Coli Bacteria ( 51,6 % ) as the most prevalent causing bacteria. The prevalence of UTI among the B and AB blood groups differed from that of the A and 0 blood groups ( 48,45 % : 27,45% ) at a significant difference of p < 0.002. ISK merupakan penyakit infeksi yang masih banyak sekitar 5 - 6 % dari seluruh populasi umum, dijumpai baik didalam praktek sehari-hari maupun di RS, dan menyerang pada semua orang tanpa tergantung suku bangsa dan daerah tempat tinggal tetapi lebih tergantung pada umur dan jenis kelamin. Diagnosis ISK ditegakkan dengan mengacu kriteria Kass yaitu bakteriuria bermakna > 105 CFU / ml dan leukosituria > 5 / LPB untuk laki - laki dan > 15 / LPB untuk perempuan, pada penderita tanpa keluhan klinik. Diduga bakteri penyebab ISK pada tahun pertama kehidupannya mempunyai antigen yang dapat mengadakan reaksi silang dengan golongan darah, yang mengandung isoaglutinin anti B. Tidak adanya isoaglutinin anti B dan adanya kemiripan struktur antigen kuman E.Coli penyebab ISK terbanyak dengan antigen golongan darah B dan AB menyebabkan resiko ISK yang lebih tinggi pada golongan darah B & AB dibandingkan A & 0. Dilakukan penelitian di Instalasi Patologi Klinik RSUP Dr. Kariadi Semarang menggunakan metode "observasional analitik" dengan pendekatan belah lintang, dalam jangka waktu 5 bulan dari bulan Juli 1997 sampai dengan bulan November 1997 mengenai resiko ISK pada golongan darah B dan AB. Dengan mengambil 336 sampel urin leukosituria tanpa keluhan klinik pada penderita laki-laki dan perempuan dari semua golongan darah ABO berusia 15 - 50 tahun' kemudian dilakukan pemeriksaan biakan kuman. Dari pemeriksaan tersebut didapatkan sebanyak 214 kelompok ISK negatif ( 63,7 % ) dan 122 kelompok ISK positif ( 36,3 % ) dan hasil rerata umur ISK lebih banyak dijumpai pada usia produktif ( 40 tahun ). Dad kelompok ISK didapatkan hasil pemeriksaan terbanyak yaitu pada golongan darah B ( 48,4 % ) dan golongan darah AB ( 48,5 % ), dengan bakteri penyebab ISK terbanyak adalah bakteri usus gram negatif terutama kuman E.Coli ( 51,6 %). Prevalensi ISK pada golongan darah B dan AB berbeda dibandingkan dengan golongan darah A dan 0 ( 48,45 : 27,45 ) dengan perbedaan bermakna p < 0,002.
