RESTON FAAL P'ArrZtj SETELAH PEMBERIA1N STUDI PADA PENDERITA USIA 40 TAHUN KK ATAS YANG MENJALANI OPERASI ABDOMEN BAGIAN BAWAH

Abstract

The effect of giving aminophyllin against the result of lung test to the patients above 40 years old who take operation at the lower part of their abdomen has been evaluated in 40 patients divided into the group of control and aminophyllin proved to have same numbers. The objective of this research is to find out wether the giving of aminophyllin supositoria 4-6 mg/kg.BW of pre-operation is able to improve the result of lung test to the said patients. The research will include the clinic research with .pre test-post test control group design and will be done at the Central Surgery Installation of Dr. Kariadi Hospital from May to july 1998 by using matching technique to equalize the condition of both groups. This being selected, the patients were cheked their result of lung test which consisting of tidal volume (TV), inspiratory reserve volume (IRV), expiratory reserve volume (ERV), force ekspiratory volume 1 sec (FEV1) dan vital capacity (VC). Aminophyllin supositoria 4-6 mg/kg. BW 5 minutes was given after induction to the group of aminophyllin. Both groups got induction of propofol 2 mg/kg. BW, vecuroniun 0,1 mg/kb. BW and enfluran oxygen and N20 was given for maintenance. At the end of operation, reserve with sufas atropin and neostig the result of lung test after oepration was done. The result of this observation was compared and made statistical test using T-test with degree of means 5% (FD. 0,05). Deficiency of obstrutive lung was found out to 10% of this research subyects with FEV1/VC between 63% to 69% . The result of lung test after operation to both groups showed declining compared to the result of lung test pre-operation. The group of control declined between 13% - 24% while the aminophyllin group between 11% - 16%. This distinction was not statisticallya difference of means for TV, IRV,ERV and VC. While the difference of means was found at FEV1. This research shows that the giving of aminophyllin supositoria pre-operation to the patients above 40 years old who take operation at the lower part of their abdomen is able to heal FEV1. Pengaruh pemberian aminofilin terhadap hasH tes faal paru (TFP) pada penderita 40 tahun keatas yang menjalani operasi abdomen bagian bawah telah diteliti pada 40 penderita yang terbagi dalam kelompok kontrol dan aminofilin sama banyak. Tujuan penelitian ini untuk mengetahui apakah pemberian aminofilin supositoria 4-6 mg/kgBB pre operasi mampu memperbaiki hasil TFP penderita tersebut diatas. Penelitian ini termasuk penelitian klinik dengan rancangan pre test post test control group design, dilakukan di Instalasi Bedah Sentral RS Dr Kariadi selama bulan Mei — Juli 1998, menggunakan tehnik metching untuk penyetaraan kondisi kedua kelompok. Setelah dilakukan seleksi penderita diperiksa TFP yang terdiri dad tidal volume (TV), inspiratory reserve volume (IRV), expiratory reserve volume (ERV), force ekspiratoiy volume I sec (FEV-I) dan vital capacity (VC). Pada kelompok arninofilin diberikan aminofilin supositoria 4-6 mg/kgBB 5 menit setelah induksi. Kedua kelompok mendapat induksi PropofoI 2 mg/kgBB, Vekuronium 0,1 rng/kgBB dan untuk rumatan diberikan enfluran oksigen dan N20. Pada akhir operasi dilakukan reverse dengan sufas atropin dan neostigmin. Setelah penderita sadar penuh dilakukan TFP pasca operasi. HasH pengamatan dibandingkan dan dilakukan uji statistik menggunakan uji beda rerata (T-tes) dengan derajat kemaknaan 5% (Psz 0,05). Ditemukan kelainan paru obstrutif pada 10% subyek penelitian dengan FEV1NC antara 63%-69%. Hasil TFP pasca operasi pada kedua kelompok mengalami penurunan dibandingkan dengan TFP pre operasi. Kelompok kontrol menurun antara 13% — 24% sedangkan pada kelompok aminofilin antara 11% - 16%. Perbedaan tersebut secara statistik tidak berbeda bermakna untuk TV, IRV, ERV dan VC. Sedangkan pada FEV1 terdapat perbedaan yang bermakna. Penelitian ini menunjukkan bahwa pemberian aminofilin supositoria pre operasi pada penderita usia 40 th. ke atas yang menjalani operasi abdomen bagian bawah dapat memperbaiki FEV1.
