ANALISIS BIAYA PELAYANAN RAWAT JALAN, OPERASI DAN REFRAKSI DJ BALAI PENGOBATAN MATA " KAMANDAKA " PURWOKERTO KABUPATEN BANYUMAS TAHUN 2001

Abstract

Tuntutan masyarakat akan pelayanan kesehatan yang bennutu semakin meningkat dan merupakan sesuatu hal yang harus diwujudkan. Untuk mewujudkan pelayanan kesehatan yang bennutu konsekuensi yang harus ditempuh adalah harus memenuhi kebutuhan operasional, melengkapi peralatan medis maupun non medis dan meningkatkan profesionalisme tenaga yang memberikan pelayanan. Keadaan sekarang dimana tarif pelayanan kesehatan masih sangat rendah tidak memungkinkan untuk mewujudkan tuntutan masyarakat akan pelayanan kesehatan yang bermutu. Masalah penelitian yang diangkat adalah berapa besar biaya satuan (Unit Cost) pelayanan rawat jalan, operasi dan refraksi dan berapa besar CRR (Cost Recovery Rate) tarif pelayanan serta faktor-faktor yang menjadi pendukung dan penghambat penetapan tarif berdasarkan anal isis biaya. Penelitian ini adalah jenis penelitian deskriptif evaluatif, yang bertujuan untuk mengetahui berapa besar biaya satuan (Unit Cost) pelayanan rawat jalan, operasi dan refraksi, juga untuk mengetahui tingkat pemulihan tarif yang berlaku terhadap biaya satuan aktual (Unit Cost), (CRR=Cost Recovery 1?ate) di Balai Pengobatan Mata "Kamandaka" Purwokerto. Metode perhitungan analisis biaya satuan yang digunakan adalah metode distribusi ganda (Double Distribution). Hasil penelitian di dapat biaya satuan (Unit C'ost) aktual rawat jalan sebesar Rp.6.732,40 (CRR=59,41 %), biaya satuan (Unit Cost) operasi sebesar Rp.632.586,89 (CRR=88,52 %) dan biaya satuan (Unit Cost) refraksi sebesar Rp. 11.046,54 (CRR=13,57 %). CRR gabungan (rata-rata) sebesar 53,83 % sehingga dapat disimpulkan bahwa selama tahun 2001 kegiatan rawat jalan, operasi dan refraksi di Balai Pengobatan Mata "Kamandaka" Purwokerto masih memerlukan subsidi dari Pemerintah Daerah sebesar 46,17 %. Dari hasil perhitungan biaya satuan (Unit Cost) pelayanan rawat jalan, operasi dan refraksi didapatkan hasil Cost Recovery Rate yang masih rendah. Agar Balai Pengobatan Mata "Kamandaka" Purwokerto dapat memenuhi kebutuhan operasional dan dapat melakukan pengembangan sena peningkatan pelayanan diperlukan sumber dana yang mencukupi. Penyesuaian tarif merupakan altematif yang sangat tepat dilaksanakan dengan tujuan untuk meningkatkan pelayanan yang berkualitas dan berkesinambungan sebagai bentuk tanggung jawab bersama antara pemerintah dan masyarakat.Diperlukan upaya advokasi dengan Stake holder (DPRD, Bupati) untuk peningkatan tarif dan mewujudkan Balai Pengobatan Mata "Kamandaka" Purwokerto menjadi unit swadana serta dalam rangka mengembangkan usaha untuk revenue generating seperti pelayanan kaca mata (optik) dan pelayanan obat-obatan (apotek) Kata kunei : Analisis biaya, Ekonomi kesehatan. To realize this, we should take the consequences of providing the operational needs, medical or non-medical tools and developing professionalism of all parties related to this service. Nowadays, where the cost of health service is still low, it is impossible to realize the demand of having a good health care service. The problems of this research are how much is the unit cost of house recovery service, operation and refraction, and how much is Cost Recovery Rate of service cost and all factors supporting and counteracting to determine the cost commencing analysis. The aims of this descriptive evaluative research are to search how much the unit cost of house recofery, operation, and reflection, to search the level of recovery cost to actual unit cost, Cost Recovery Rate on Kamandaka eyes clinic, Purwokerto. The unit cost analysis method used is Double Distribution Method. The result of thle research state that the actual unit cost of house recovery is Rp. 6.732,40 (CRR = 59,41%), operation unit cost is Rp. 632.586,89 (CRR = 88,52 %) and refraction unit cost is Rp. 11.046,54 (CRR = 13,57%). Average CRR is 53,83%. The conclusion is that during 2001, house recovery, operation, and refraction activities on Kamandaka eyes clinic, still need to be subsidiesed 46,17 % by local government.From the account of house recovery, operation, refraction unit cost shall be concluded that the cost recovery rate is still low. In order to provide all the operational need, development and service, Kamandaka eyes clinic needs enough funds. Tariff adjushnent is the proper alternative way to develop and maintain service quality as the responsibility of people and government. It need many efforts to advocate the stakeholder (DPRD, Bupati) in developing tariff and making this eye clinic to be able to provide all the need and developing revenue generating like glasses and drugs services.
