Abstract
The JPKM participation in Purbalingga Regency succeeded in recruiting more participants than the JPKM participation in Tegal Regency, which was 67,560 participants, while in Tegal Regency there were only 299 participants. The purpose of this research was to find out the strenghts and the superiorities of the JPKM implementation through benchmarking in Purbalingga Regency especially in the case of the participatory management, the financial management, the health maintenance management, the Bapel JPKM institution management, and the role and function of Bapim JPKM (Tri Karya JPKM). It was hoped that from the benchmarking result a recommendation can be arranged in order to develop the JPKM in Tegal Regency. This research was a non-experimental research with a descriptive explorative scheme. This research was an interpretative research with a qualitative approach. The data was collected through interviews and Focused Group Discussions (FGDs) and then it was analysed descriptive qualitatively, and hypothesis test was not done. Benchmarking was a comparative and differential system of a health service process. Since benchmarking had nothing to do with figures and statistics, it was more proper done through a descriptive process study. The benclunarking result of the JPKM implementation in Purbalingga Regency showed that the JPKM could develop well because compared with the one in Tegal Regency, it has several superiorities, among others things are it has a participant recruitment mechanism, a regulation support from the district government and the legislative, the legal status of incorporated (Inc) Badan Penyelenggara (Bapel) and has a premium subsidy for the poor family. Based on the streghts and the superiorities above, a recommendation regarding the development of the JPKM in Tegal Regency had been arranged, those were : the improvement of the participant recruitment system by putting cooperation with NGO and labour association, making a regulation about the JPKM implementation in the form of SK Bupati or Perda JPKM, the calculation of the participant premium which pay more attention to the will and the ability of the society, the need of the transferring of the Bapel JPKM legal status from under the KPRI Bakti Husada autonomy into incorporated (Inc.) for increasing the autonomous JPKM, the support, participation and commitment of the JPKM top leader and performers, the repairing of the Bapel JPKM human resources and for faising the JPKIvI participation scope for example for the village leader and its assistants. Conclusion : The JPKM performers in Tegal Regency has to increase their support, commitment and hardwork to develop the JPKM in the future. An experience in Purbalingga Regency showed that through cooperation and participation from all sectors, the development of JPKM was going easier and better. Kepesertaan JPKM di Kabupaten Purbalingga telah berhasil menjaring peserta lebih banyak dari kepesertaan JPKM di Kabupaten Tegal, yaitu sebanyak 67.560 peserta sementara di Kabupaten Tegal baru 299 peserta. Tujuan dari penelitian ini adalah untuk mengetahui kekuatan dan keunggulan dalam penyelenggaraan JPKM melalui benchmarking di Kabupaten Purbalingga, terutama dalam hal manajemen kepesertaan, keuangan, pemeliharaan kesehatan, institusi Bapel JPKM dan peran dan fungsi Bapim JPKM (Tri Karya JPKM). Diharapkan dari hasil benchmarking ini dapat disusun rekomendasi dalam mengembangkan JPKM di Kabupaten Tegal. Penelitian ini merupakan penelitian non eksperimen dengan rancangan deskriptif eksploratif. Penelitian ini bersifat intepretatif dengan pendekatan kualitatif. Data diperoleh melalui wawancara dan Diskusi Kelompok Terfokus (DKT) selanjutnya dianalisa secara deskriptif kualitatif, tetapi tidak dilakukan uji hipotesis. Benchmarking adalah suatu sistem perbandingan dan perbedaan dari suatu proses pelayanan kesehatan, karena benchmarking tidak berhubungan dengan angka atau statistik, maka akan tepat melalui kajian deskripsi proses. Basil benchmarking penyelenggaraan JPKM di Kabupaten Purbalingga, diketahui bahwa JPKM dapat berkembang dengan baik karena mempunyai keunggulan dan kekuatan dari JPKM di Kabupaten Tegal, diantaranya mempunyai mekanisme perekrutan peserta, dukungan regulasi dari Pemda dan legislatif, status badan hukum Bapel JPKM berupa PT, dan adanya subsidi premi bagi keluarga miskin. Berdasarkan keunggulan dan kekuatan tersebut di atas, telah disusun rekomendasi dalam pengembangan JPKM di Kabupaten Tegal, yaitu: perbaikan pola rekrutmen peserta dengan menggalang kedasama dengan LSM dan asosiasi pekerja, membuat aturan penyelenggaraan JPKM yang berupa SK Bupati atau Perda JPKM, penghitungan premi peserta yang lebih memperhatikan kemauan dan kemampuan masyarakat, perlunya pengalihan status badan hukum Bapel JPKM dad di bawah otonomi KPRI Bakti Husada ke bentuk PT untuk peningkatan JPKM mandiri, peningkatan dukungan, partisipasi dan komitmen dari Top leader dan pelaku JPKM, pembenahan SDM Bapel JPKM dan peningkatan cakupan kepesertaan JKIYI umpamanya untuk Keala Desa dan perangkatnya Kesimpulan : Bagi para pelaku JKM di Kabupaten Tegal dalam mengembangkan JPKM di masa mendatang agar lebih meningkatkan dukungan, komitmen, partisipasi dan kerja kerasnya. Pengalaman di Kabupaten Purbalingga menunjukkan dengan keriasama dan partisipasi dari seluruh sektor yang ada pengembangan JPKM menjadi lebih mudah dan berialan lebih baik.
