Abstract

Obiedive. To evaluate the efficacy and safety of two ginger species extract, Zingiber officinale and Kaempferia galanga compare to acetaminophen, in osteoarhthritis (OA) of the knee patients. Methods. Fourty-six patiens with OA of the knee by the Altman 1991 criteria and grade 1, grade 2, and grade 3 by the Kellgren-Lawrence criteria were enrolled in a randomized, double-blind, controlled study for 14-days. After washout period, the patients were received ginger extract or acetaminophen three times daily. The outcome variable were reduction in knee pain (on a visual analog scale / VAS) on resting, walking (10 m), sitting to standing, climbing up and down the stairs, pressing on the knee, and the improvement stiffness of the joint, the range of motion (ROM), diameter of the knee, Lequesne index. Statistic evaluated by student's t test . The adverse event were evaluated include simptoms and change of laboratory value. Efficacy and safety assesments were performed at baseline and after days 7 and 14 of treatment. Results. After 7 days of treatment, the parameters of the pain and the inflammation were not significant difference between two groups, except in the improvement of pain of climbing up and down the stairs, the acetaminophen group was superior (P-4,003). After 14 days of treatment the ginger extract group was superior to improve the parameters of the inflammation ; stiffness of the joint (11-3,018), ROM (P3,002), diameter of the knee (P--,002) and Legume index (160,006). There was only one patient in the ginger extract group who complianced nausea No change significant the laboratory value on two groups. Conclusion. The ginger extract group was superior to improve the inflammation after 14 days of treatment. There was not significant difference in reducing of the knee pain between the ginger extract and the acetaminophen group. There was a safety profile, with mostly mild gastrointestinal adverse event in the ginger extract group.
