FAKTOR-FAKTOR RISIKO KEJADIAN ARTRITIS LEPRA DI DONOROJO KELING JEPARA

Abstract

Background : Leprosy is a chronic infections disease caused by Mycobacterium leprae. Considerabel attention has been devoted to the dermal, neural and osseous complications of leprosy. Reports of joint involvement in leprosy have been published since 1960s. However, only a few studies have highlighted the clinical pattern and risk factors of joint involment in leprosy. An inflammatory polyarthritis has been previously described in leprosy but accounts of the clinical characteristics have varieted. Material and Methods : A control case study was conducted to study the risk factors of arthritis in leprosy patients of Donorojo Hospital, Jepara. Data was collected from September up to November 2003. 30 cases arthritis in leprosy and 30 controls leprosy patients without arthritis, same in age and set. The risk factors of arthritis in leprosy are (1) history of family, (2) status of therapy, (3) leprosy reactions, (4) history of ill. The statistic methods is done with SPSS program. The data analysis with Chi-square and Fisher exact test. Results : 30 patients arthritis in leprosy patients, ages means of 50,6 ± 12,34 years. History of ill is the most important risk factors with the highest odds ratio (OR=7.0, p=0.01). Latar belakang : Penyakit lepra adalah penyakit infeksi kronik yang disebabkan oleh kuman mikobalcterium lepra. Perhatian yang besar akibat komplikasi penyakit lepra selalu ditujukan pada kelainan yang mengenai kulit, saraf dan tulang-tulang. Laporan-laporan tentang keterlibatan sendi telah dilaporkan sejak tahun 1960-an. Akan tetapi, hanya sedikit penelitian-penelitian tersebut yang menjelaskan gambaran klinik dan faktor-falctor risiko keterlibatan sendi pada penyakit lepra. Poliartritis inflamasi yang menjelaskan sebelumnya kejadian artritis pada lepra ternyata gambaran kliniknya bervariasi. Bohan dan cam : Jenis penelitian adalah kasus kontrol untuk mengetahui faktor¬faktor risiko artritis lepra pada pasien-pasien lepra yang dirawat di Rumah Sakit Donorojo Jepara. Data diambil mulai bulan September sampai Nopember 2003. 30 orang kasus dan 30 kontrol tanpa artritis, usia dan tempat tinggal sama. Faktor¬faktor risiko artritis lepra adalah (1) riwayat keluarga,,(2) status pengobatan (3) reaksi lepra, (4) lama sakit. Perhitungan statistik dengan menggunakan SPSS. Analisa data dengan Chi-square dan Fisher's exact test. Hasil : 30 kasus artritis lepra, usia rerata 50,6 ± 12,34 tahun. Lama sakit merupakan faktor risiko yang paling penting dengan rasio odd (R0=7.0, p=0.01).
