Abstract
BACKGROUND: Polycystic ovary syndrome (PCOS) occurs 5-10% among woman of reproductive age. Its major clinical manifestation is anovulation, infertility, and hyperandrogenism. The etiology and pathogenesis of PCOS are still obscure. Recent investigations have shown that the insulin resistance contributes to the major caused of PCOS. OBJECTIVE: The aim of this study is to investigate the insulin resistance frequency by means of HOMA-IR method in PCOS patients at RSIA Anugerah Semarang, and to find correlation of pCOS patients with obesity and the change of her androgen hormone. MATERIAL AND METHOD: This was cross sectional study. Patients with PCOS at Anugerah Hospital Semarang were studied. The subjects were treated by standard procedures such as: anamnesis, physical examination and laboratory test included fasting blood glucose, fasting insulin, androstenedion, estradiol and free testosterone hormones. Laparoscopy examination was utilized to diagnose the polycystic ovary. The diagnosis of PCOS was carried out according to The Rotterdam ESHEE/ARM 2003 criteria. The insulin resistance was calculated by HOMA-IR formula. The relation among the variables was determined by means of statistical analysis of Chi-square and Fischer exact test. RESULTS: We have studied 35 cases of PCOS, 82,9% was insulin resistant (+) and 17,1% insulin resistant (-). In this study, we found 25,7% was obese and 74,3% was non obese. The subjects showed an increment of androstenedion and free-testosterone hormones of 34.5% and 17.1%, respectively. In the group of insulin resistant (+): 89.7% was non-obese and 10.3% was obese. In this group, the subjects showed an increment of androstenedion and free testosteron hormones of 37.9% and 17,2%, repectively. While in the group of insulin resistant (-), 85.7% was non-obese and 14.3% was obese. In this group, the subjects showed an increment of androstenedion and free testosteron hormones of 16.7%. CONCLUSION: This study concludes that insulin resistance frequency in PCOS patients obtained by means of HOMA-IR method is 82.9%. There is no different in insulin resistance frequency between obese and non-obese PCOS were found. Furthermore, a correlation study does not show a significant difference between insulin resistant and the change of androstenedion and free-testosterone hormones in PCOS patients. LATAR BELAKANG : Sindroma Ovarium Polikistik ( SOPK ) diderita oleh 5 — 10% wanita usia reproduksi. Kelainan utamanya berupa anovulasi, infertilitas dan gejala hiperandrogenisme. Etiologi dan patogenesis SOPK belum pasti. Salah satu penyebab SOPK yang banyak dikemukakan akhir-akhir ini adalah resistensi insulin. TUJUAN : Penelitian ini ditujukan untuk mengetahui frekuensi resistensi insulin dengan metode HOMA-IR pada penderita SOPK di RSIA Anugerah Semarang, serta hubungannya dengan obesitas dan perubahan hormon androgen yang terjadi. BAHAN DAN CARA : Disain penelitian adalah studi potong lintang. Subjek penelitian adalah penderita SOPK di RSIA Anugerah Semarang. Pada penderita yang memenuhi kriteria, dilakukan anamnesis, pemeriksaan fisik dan pemeriksaan laboratorium yang meliputi gula darah puasa, insulin puasa, hormon androstenedion dan free testosteron. Pemeriksaan laparoskopi dilakukan untuk mendiagnosis Ovarium Polikistik. Diagnosis SOPK menggunakan kriteria menurut kriteria The Rotterdam ESHEE/ARA tahun 2003. Perhitungan resistensi insulin menggunakan rumus HOMA-IR. Dilakukan analisis statitistik dengan Chi-square test atau Fischer exact test untuk mengetahui hubungan/perbedaan antar variabel. HASIL : Didapatkan 35 kasus SOPK, 82,9% dengan resistensi insulin (+) dan 17,1% resistensi insulin (-). Pada studi ini didapatkan 25,7% pasien SOPK obes dan 74,3% non obes. Subjek yang menunjukkan peningkatan hormon androstenedion dan free testosteron sebesar 34,5% dan 17,1%. Pada kelompok resistensi insulin (+): 89,7% non obes dan 10,7% obes. Pada kelompok hil, subjek yang menunjukkan peningkatan hormon androstenedion dan free testosteron 37,9% dan 17,2%. Sedangkan pada kelompok dengan resistensi insulin (-); 85,7% non obes dan 14,3% obes. Pada kelompok ini, subjek yang menunjukkan peningkatan hormon androstenedion dan free testosteron masing-masing sebesar 16,7%. KESIMPULAN : Frekuensi resistensi insulin dengan metode HOMA-IR pada penderita SOPK adalah sebesar 82,9%. Tidak didapatkan hubungan yang bermakna antara resistensi insulin dengan obesitas pada penderita SOPK. Tidak didapatkan hubungan yang bermakna antara resistensi insulin terhadap perubahan hormon androstenedion dan free testosteron pada penderita SOPK.
