Abstract

Backgroud: The use of ketamine has been widely accepted with all the advantages and disadvantages. Ketamine can be used for high risk patient in whom hemodynamic and respiratory stability are needed. Ketamine must be used with caution because of it's adverse effect i.e. sympatomimetic effect, phsycotomimetic reaction and blood glucose increase. The study of ketamine dosage increase toward blood glucose was based by this hyperglicemic effect. Objective: To find objective evidence of blood glucose increase caused by increases of ketamine dosage. Method: This study was a randomized double blind clinical trial. Forty-eight patient underwent elective surgery in Dr. Kariadi General Hospital, Semarang, age 14-40 year-old, physical status ASA I-Il, fulfill inclusion criteria, were divided into two groups. Group I received induction dosage of ketamine 1 rng/kgBW and group II received 2 mg/kgBW. The change of blood glucose were measured at 15, 30, 45 and 60 minute. Statistic test used were chi-square and t-test. Result: Characteristic data were not differ significantly between two groups. Blood glucose increase were found in both groups. The biggest increase was found in first 15 minutes, 16% increase in group I and 21% increase in group II. Blood glucose increase caused by increase of ketamine dosage were not differs significantly between two groups (p>0,05). Conclusion: Increase of ketamine dosage was not resulted in blood glucose increase. Latar Belakang: Penggunaan ketamin telah diterima secara luas dengan segala kelebihan dan kekurangannya. Ketamin dapat dipergunakan pada penderita dengan resiko tinggi yang membutuhkan kestabilan hemodinamik dan respirasi. Tetapi penggunaan ketamin harus hati-hati karena kekurangan yang dimilikinya, misalnya efek simpatomimetik, reaksi psikotomimetik dan peningkatan kadar glukosa darah. Karena pengaruhnya terhadap terjadinya hiperglikemia inilah yang mendasari penelitian peningkatan dosis ketamin terhadap kadar glukosa darah. Tujuan : Mendapatkan bukti obyektif peningkatan dosis ketamin akan menyebabkan peningkatan kadar glukosa darah. Metode: Penelitian ini dirancang sebagai uji klinis acak tersamar ganda terhadap 48 penderita yang menjalani operasi elektif di RSUP Dr. Kariadi Semarang, usia 14 40 tahun, status fisik ASA I-II dan memenuhi kriteria inklusi. Penderita dibagi dua kelompok, yaitu kelompok I mendapatkan ketamin dosis induksi 1 mg/kgBB dan kelompok II mendapatkan ketamin dosis induksi 2 mg/kgBB. Kemudian kedua kelompok diukur perubahan kadar glukosa darah setiap menit ke 15, 30, 45 dan 60. Uji statistik dilakukan dengan Chi-square test dan Nest. Basil: Data karakteristik penderita tidak didapatkan perbedaan yang bermakna diantara kedua kelompok penelitian. Peningkatan kadar glukosa darah didapatkan pada kedua kelompok penelitian. Peningkatan terbesar didapatkan pada 15 rnenit pertama, yaitu sebesar 16% pada kelompok I dan sebesar 21% pada kelompok II. Tetapi peningkatan dosis tidak menyebabkan terjadinya peningkatan kadar glukosa darah karena dari hasil penelitian tidak didapatkan perbedaan yang bermakna (p>0,05) antara kedua kelompok penelitian ini. Kesimpulan: Peningkatan dosis ketamin tidak menyebabkan peningkatan kadar glukosa darah.
