Abstract

Triamcinolone acetonide ( TA ) has frequenly been used as a keloid therapy that was thought to have an inhibition on collagen synthesis and fibroblast proliferation. But up till now there is no satisfying result on keloid therapy. So that in this study we tried a combination of TA and 5FU in the hope to increase the effectiveness by doing the Fibrobla•t Populated Collagen Lattice ( Fpa ) and Agyrophylic Nucleolar Organizer Region (AgNOR ) tests. This laboratory-experimental study was designed as a Post test-only control group using cultured fibroblast cells that was cultured in wells as experimental objects. All samples were divided into twelve t•eatment groups consisted of control group ( TA-5FU- ); single-exposured treatment groups ( TA10 5FU-, TA20 5FU-, TA-5FU0,1, TA-5FU1, TA-5FUl ) and combination-exposured treatment groups (TA20 5FU10 ). FPCL inhibition was assessed using Scion Image and proliferation index was assessed based on the average of NOR on 100 cells. Experiment was done triplo. It was shown in this study that there were significant differences on FPCL inhibition and proliferation index between single-exposured treatment groups with control group, FPCL inhibition between combirtation-exposured treatment groups with control group, between combination-exposured treatment groups with single-exposured treatment groups but there were no significant differences on proliferation index between TA single-exposured treatment groups with control group, between TA-5FU0,1 treatment groups and TA-5FUl but there was a significant difference on proliferation index between TA-5FU0,1 and TA-5FU10.. It can be concluded that the combination-exposure of TA and 5FU could increase collagen synthesis inhibition significantly that was in proportion with the drug dose but there was no significant difference on proliferation index between combinationTA and 5FU with single 5FU , yet there was a significant difference with single TA. Triamsinolon asetoind ( TA ) sering dipakai untuk pengobatan keloid yang diduga bekeija dengan rnengharnbat sintesis kolagen dan proliferasi fibroblas. Tetapi sampai saat min belurn ada pengobatan keloid yang hasilnya memuaskan sehingga pada penelitian ini dicobakan kombinasi TA dan 5 Fluorourasil ( 5FU ) dengan harapan dapat meningkatkan efekriya dengan uji Fibroblast Poptdated Collagen Lattice ( PPCL ) dan Agyrophylic Nucleolar Organizer Region (AgNOR) Penelitian fin merupakan penelitian eksperimental laboratorik dengan rancangan The Post Te•t-Only Control Group De•ign yang menggunakan sel-sel fibroblas yang dikultur dalam sumuran sebagai obyek penelitian. Sampel dikelompokkan menjadi 12 kelompok perlalmn yang terdiri dad kelompok perlakuan kontrol ( TA-5FU- ), kelompok perlakuan pemaparan tunggal (TAIO 5FU-, TA20 5FU-, TA-5FU0,1, TA-5FU1, TA-5FU10 ) dan kelompok perlakuan pernaparan kombinasi ( TA20 5FU10 ). I-lambatan FPCL dihitung dengan Scion Image dan Indek Proliferasi dihitung berdasarkan rerata jumlah NOR dalam 100 sel. Percobaan dilakukan secara triplo. Hasil penelitian menunjukkan adanya perbedaan hambatan FPCL dan indek proliferasi antara kelompok perlakuan pemaparan tunggal dibandingkan dengan kelompok kontrol, adanya perbedaan hambatan FPCL antara kelompok perlakuan pemaparan koinbinasi dengan kelompok kontrol, adanya perbedaan hambatan FPCL antara kelompok perlakuan pemaparan kombinasi dibandingkan kelompok perlakuan tunggal tetapi tidak ada perbedaan indek proliferasi yang bennakna secara statistik antara kelompok perlakuan pemaparan TA tunggal dibandingkan kelompok kontrol, antara kelompok perlakuan TA-5FU0,1 dan TA-5FU1 tetapi terdapat perbedaan yang bertnakna secara statistik antara indek proliferasi TA-5FU0,1 dan TA-5FU10. Sehingga dapat disimpulkan bahwa pemaparan kombinasi TA dan 5FU dapat meningkatkan pengharnbatan sintesis kolagen secara bermakna yang berbanding lurus dengan dosis obat tetapi efek penurunan indek proliferasi kombinasi TA dan 5FU tidak berbeda secara bennakna dengan 5FU tunggal, dan berbeda secara bermakna dengan TA tunggal.
