LAJU ENDAP DARAII, C-REAKTIF PROTEIN, DAN ALFA 2-MAKROGLOBULIN SEBAGAI FAKTOR RISIKO ARTRITIS LEPRA DI RS KUSTA DONOREJO JEPARA

Abstract

Background: Leprosy is a chronic infectious disease, caused by Mycobacterium leprae, It has various clinical features depending on the immune responses of the patients, mainly humoral immune response. The erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and alpha-2 macroglobulin (cc-2M) are some of the humoral immune responses of the leprosy patients. TH-1 and TH-2 lymphocyte function changes is suspected as the basic pathogenesis of arthritis in leprosy. Methods: The purpose of this study is to find out the risk factors of leprosy arthritis in leprosy patients that underwent treatment in the Leprosy Hospital of Donorejo, Jepara. This is a case-control study which is involved 30 case patients of leprosy arthritis and 30 control patients i.e. leprosy patient without arthritis. Age, sex and residence of the cases and control patients are similar. Data collection was performed from September to November 2003. The risk factors of leprosy arthritis being studied are ESR, CRP, and a-2M. The statistic methods is done with SPSS program The data analysis with Chi-Square and Fisher Exact Test. Results: The case group and the control group each consists of 13 (43.3%) males and 17 (56.7%) females. The average age of the case group is 50.63 years and that of the control group is 49.37 years. Patients with leprosy arthritis have ESR and CRP level which are significantly higher than leprosy patients with have no arthritis. HigKESR was found in 27 case patients and in 17 control patients with OR (odds ratio) of 6.50; p = 0,002; 95% CI, 1.82 - 23.21. High CRP level was found in 16 case patients and in 2 control patients with OR (odds ratio) of 16.00; p = 0,000; 95% CI, 3.22 - 79.56. Whereas high a-2M level was found in 2 case patients and in 2 control patients with OR (odds ratio) of 1.00 ; p = 1.000 ; 95%CI (0.16 — 5.40). Conclusions: The Erythrocyte sedimentation rate (ESR) and level of C-reactive protein (CRP) are risk factors for leprosy arthritis, whereas alpha 2 macroglubulin a-2M is not a risk factor for leprosy arthritis. Latar belakang :Penyakit lepra adalah penyakit infeksi kronik yang disebabkan oleh Mycobacterium leprae dengan gambaran klinik yang bermacam-macam tergantung respon imun penderita, terutama respon imunitas humoral. Laju endap darah (LED), C-reaktif protein (CRP), dan alfa 2 makroglobulin (a-2M) merupakan gambaran sebagian dari respon imunitas humoral penderita lepra. Perubahan fungsi limfosit TH-1 dan Th2 diduga merupakan dasar patogenesis pada kelainan arthritis lepra. Metoda : Tujuan penelitian ini adalah untuk mengetahui falctor risiko artritis lepra pada penderita lepra yang dirawat di Rumah Sakit Kusta Donorejo Jepara. Jenis penelitian ini adalah kasus kontrol dengan melibatkan 30 orang kasus penderita artritis lepra dan 30 orang kontrol yaitu penderita lepra tanpa artritis. Usia, jenis kelamin dan tempat tinggal baik pada kasus maupun kontrol sama. Pengambilan data dilakukan mulai bulan September sampai dengan Nopember 2003. Faktor risiko artritis lepra yang diteliti adalah LED, CRP, dan a-2M. Perhitungan statistik dikerjakan dengan menggunakan program SPSS. Analisa data dengan Chi-Square dan Fisher's Exact Test. Hasil : Kelompok kasus dan kontrol masing-masing terdiri dari 13 (43,3%) lakilaki dan 17 (56,7%) perempuan. Usia: rata-rata kelompok kasus 50,63 tahun dan kelompok kontrol 49,37 tahun. Penderita artritis lepra menunjukkan laju endap darah dan kadar C-reaktif protein yang secara signifikan lebih tinggi daripada penderita lepra tanpa disertai artritis. LED yang tinggi didapatkan pada 27 kasus dan 17 kontrol dengan Odds Ratio (OR) = 6.50 ; p = 0,002; 95% CI = 1.82 —23.21. Kadar CRP yang tinggi didapatkan pada 16 kasus dan 2 kontrol dengan Odds Ratio (OR) = 16.00; p = 0.000; 95% CI = 3.22 — 79.56. Sedangkan kadar a-2M tinggi didapatkan pada 2 kasus dan 2 kontrol dengan Odds Ratio (OR) = 1.00 p=1.000 ; 95% CI = 0.19 — 5.40.Kesirupulan Laju endap darah (LED) dan c-reaktif protein (CRP) merupakan faktor risiko artritis lepra, sedangkan alfa 2 makroglobulin tidak merupakan faktor risiko artritis lepra
