Abstract
The oral mucus ferning test as the alternative technique for cervical mucus ferning in examining the women hormonal status maybe important. The study was conducted to determine the oral mucus compound, especially related to the the structure of the ferning (feming test). The objective of this study was to compare the composition of salt and "ferning - pattern" between the cervical and oral mucus, by cross - sectional design. The subject of this study was 30 women, that were taken their cervical and oral mucus once within the 6th to 19th day of menstruation. The feming test and Na level of cervical mucus were analyzed, as well as feming test, Na and K levels of oral mucus. The light microscope was used in examining the "feming pattern". AAS methods was used in measuring the Na and K levels. The data distribution was analyzed by Kolmogorf Smirnov, followed by non - parametric statistics. The mean, median and standart deviation were calculated with descriptive analysis. The differentiation between group of cervical and oral mucus feming score were tested by Mann - Whitney, The Wilcoxon signed Rank test was used to analyze the differentiation between the Na and K levels of oral mucus in groups. The Spearman test was used to find the correlation between ferning score and the Na and K levels of . cervical / oral mucus. The result of this study shows a bit difference between the "feming pattern" of cervical and oral mucus. The cervical feming pattern is a fern - like leaf with a clearly difference of primary, secondary, tertiary and quartenery stem. But the oral ferning pattern is a fern - like leaf with a relatively somelike of primary, secondary, tertiary and quartenery stem, and more fragile. The Mann - Whithey test of cervical and oral mucus ferning score between groups is not significant (p=0,7), but between oral mucus Na and K levels is significant ( p=0,0001), where K level is higher than Na. The mean and standard deviation of cervical mucus Na levels in each group which have the feming score 1,2,3 are 1,98(0,000), 5,84(2,909), 46,09(2,696) consecutively. The mean and standard deviation of oral mucus K levels in each group which have the ferning score 1,2,3 are 7,62(0,630), 13,07(0,758), 50,15(4,504) consecutively. It is concluded that individually there is no difference between cervical and oral mucus ferning score in the same time, the oral mucus K level is higher than Na level, there are positive correlation between cervical mucus ferning score and Na level with correlation coefficient (Spearman's rho) 0,823 and between oral mucus feming score and K level with correlation coefficient (Spearman's rho) 0,870. Dasar pemakaian tes ferning lendir mulut sebagai pengganti tes ferning lendir serviks dalam penentuan status hormonal masih perlu dipertimbangkan. Karena itu penelitian terhadap komposisi lendir mulut terutama yang berhubungan dengan struktur daun pakis (tes ferning) layak dilakukan. Tujuan penelitian adalah membandingkan komposisi garam dan gambaran ferning antara lendir serviks dan lendir mulut ; dilakukan secara cross-sectional yang bersifat analitik. Subyek penelitian sebanyak 30 wanita, masing-masing diambil lendir serviks dan lendir mulutnya sekali pada hari ke 6 s/d 19 dari menstruasi. Kemudian dilakukan tes ferning dan analisis kadar garam Na terhadap lendir serviks serta tes feming dan analisis kadar garam Na dan K terhadap lendir mulut. Pengamatan preparat feming dilakukan dengan mikroskop cahaya path perbesaran 10x10 sedang analisis kadar garam Na dan K menggunakan metode AAS (Atomic Absorbtion Spectrofotometer). Semua data diuji normalitasnya dengan uji KolmogorfSmirnof dan dilanjutkan dengan uji non parametrik. Analisis deskriptif dilakukan dengan menghitung rerata, nilai tengah, dan simpang baku. Perbedaan antara kelompok nilai ferning lendir serviks dan nilai feming lendir mulut dianalisis dengan uji Mann-Withney sedang antara kadar garam Na dan K pada lendir mulut dengan uji Wilcoxon Signed Ranks. Analisis korelasi antara nilai ferning terhadap kadar garam Na ataupun K pada lendir serviks dan lendir mulut dilakukan dengan uji korelasi Spearman. Hasil yang didapat menunjukkan bahwa berdasarkan pengamatan mikroskopis terdapat sedikit perbedaan pola ferning antara lendir serviks dan lendir mulut. Ferning pada lendir serviks berbentuk daun pakis dengan ukuran batang primer, sekender, tersier dan kuartener yang relatif dapat dibedakan. Sedang pada lendir mulut berbentuk daun pakis tetapi ukuran batang primer, sekender, tersier dan kuartener besamya relatif sama dan secara keseluruhan nampak lebih rapuh. Perbedaan kelompok nilai ferning antara lendir serviks dan lendir mulut tidak bermakna (p=0,7). Terdapat perbedaan bermakna antara kadar garam Na dan K dalam lendir mulut (p=0,0001) dimana kadar garam K jauh lebih tinggi daripada kadar garam Na. Rerata dan simpang baku kadar garam Na lendir serviks masing-masing kelompok nilai ferning 1,2,3 berturut-turut adalah 1,98(0,000) ; 5,84(2,909) ; 46,09(2,696). Rerata dan simpang baku kadar garam K lendir mulut masing-masing kelompok nilai ferning 1,2,3 berturut-turut adalah 7,62(0,630) ; 13,07(0,758) ; 50,15(4,504). Dapat disimpulkan bahwa tidak ada perbedaan nilai ferning antara lendir serviks dan lendir mulut pada tes feming yang dilakukan terhadap satu individu pada walctu bersamaan, pada lendir mulut kadar garam K terdapat jauh lebih tinggi daripada kadar garam Na, terdapat korelasi positif antara nilai feming lendir serviks dan kadar garam Na lendir serviks dengan koefisien korelasi Spearman (rho) sebesar 0,823; terdapat korelasi positif antara nilai ferning lendir mulut dan kadar garam K lendir mulut dengan koefisien korelasi Spearman (rho) sebesar 0,870.
