Abstract

Backgrounds: Anticoagulants that can be used in the investigation of transudate exudate are Sodium Citrate 20% or EDTA. Na Sitras and EDTA have common action capture point, that is binding calcium ion. EDTA is also used in routine hematology examination because it relatively doesn't affect cellular morphology. Examination of protein and leukocyte concentration (amount and differential count) are important parameters for detennining body fluid including transudate or exudate. The present study is intended to find out discrepancies in the results of parameter investigation when Sodium Citrate 20% and EDTA are used. For the uniformity of samples, we used pleural effusion. Objectives: To find out discrepancies in the results of examination of protein and leukocyte in pleural effusions that contain anticoagulant Sodium Citrate 20% and EDTA. Methods: We used 30 samples collected consecutively from February 2003 until early August 2003, originated from patients with pleural effusions hospitalized in internal disease wards of Dr Kariadi Hospital Semarang. The samples were obtained by toracentesis, then it were stored in 2 tubes that each contained either Sodium Citrate 20% or EDTA as anticoagulant. Examination of protein concentration was performed using Biuret method. The amount of leukocyte was calculated using Improved Neubauer counting-chamber and differential count was performed on smear preparations from samples that have been stained using Giemsa. Statistical tests were performed by differential test using paired t-test and Wilcoxon signed test. Results: The results of protein examination with Sodium Citrate 20% as anticoagulant were higher as compared with EDTA (there were significant differences, with p<0,01). The amount of leukocyte in EDTA was higher than that in Na sitras (there were significant differences, with p<0,01). The results of examination in differential count of mononuclear and polymorphonuclear cells in EDTA and Na sitras have no significant difference (p>0,05). The cellular morphology in EDTA was more intact than that in Na sitras. Conclusions: The examination of protein concentration of transudate exudate using Sodium Citrate 20% as anticoagulant has obtained higher results as compared with EDTA. The examination of leukocyte amount in EDTA has obtained higher results as compared with Sodium Citrate 20%. The results of examinations in differential count of leukocyte have showed no difference between the two, but in EDTA the blood cells were relatively more intact. Suggestion: EDTA could become anticoagulant of choice in examinations of transudate exudate because it will give better results in leukocyte examinations as compared with Sodium Citrate 20%. Key words: transudate exudate, pleural effusion, EDTA, Sodium Citrate 20%, protein, leukocyte Latar belakang: Antikoagulan yang dapat dipakai untuk transudat-eksudat adalah Na Sitras 20% atau EDTA. Na Sitras 20% dan EDTA mempunyai titik tangkap kerja yang sama yaitu mengikat ion kalsium. EDTA juga dipakai dalam pemeriksaan hematology rutin karena tidak mempengaruhi morfologi sel. Pemeriksaan protein dan leukosit (jumlah dan hitung jenis) merupakan parameter penting untuk menentukan suatu cairan tubuh termasuk transudat atau eksudat. Penelitian ini dimaksudkan untuk mengetahui perbedaan basil pemeriksaan parameter tersebut bila dipakai Na Sitras dan EDTA. Untuk keseragaman sampel dipakai efusi pleura. Tujuan: Mengetahui perbedaan hasil pemeriksaan protein dan leukosit pada efusi pleura yang diberi antikoagulan Na Sitras 20% dan EDTA. Metoda: Dipakai 30 sampel yang diambil secara konsekutif mulai februari sampai awal agustus 2003, berasal dari pasien dengan efusi pleura yang dirawat dibangsal penyakit dalam RS Kariadi Semarang. Sampel diperoleh dengan torasentesis, kemudian ditampung dalam 2 tabung masing-masing diberi antikoagulan Na Sitras 20% dan EDTA. Pemeriksaan kadar protein ditentukan dengan metode Biuret. Jumlah leukosit dihitung dengan bilik hitung Neubauer Improved dan hitung jenis dilakukan pada preparat hapus dari sampel yang telah dicat dengan giemsa.Uji statistik dengan uji beda paired t — test dan Wilcoxon signed test. Hasil: Hasil pemeriksaan protein pada Na sitras 20% lebih tinggi dibanding EDTA (terdapat perbedaan bermakna, dimana p<0,01). Jumlah leukosit pada EDTA lebih tinggi dibanding Na Sitras 20% ( terdapat perbedaan bermakna dimana p<0,01). Hasil pemeriksaan hitting jenismononuklear dan polimorfonuklear pada EDTA dan Na Sitras tidak berbeda bermakna (p>0,05). Morfologi sel pada EDTA lebih utuh dibanding Na Sitras. Kesimpulan: Pemeriksaan kadar protein transudat eksudat dengan antikoagulan Na Sitras 20% memperoleh basil yang lebih tinggi dibanding EDTA. Hasil pemeriksaan jumlah leukosit pada EDTA lebih tinggi dibanding Na Sitras 20%.Hasil pemeriksaan hitung jenis leukosit tidak menunjukkan perbedaan antara keduanya, akan tetapi pada EDTA sel-sel dash relatiflebih utuh. Saran; Antikoagulan EDTA dapat dijadikan pilihan dalam pemeriksaan transudat eksudat karena memberikan basil pemeriksaan leukosit yang lebih baik dibanding Na Sitras 20%.
